When signing up to our classes you certify that the child named has your permission to participate in Skillz Skool classes. You further certify and grant permission to coaches of Skillz Skool to act for me according to their best judgment in any emergency requiring medical attention. Furthermore, I hereby waive and release Skillz Skool, its employees, agents, officers and staff for any accident or injury sustained while at this class.

Skillz Skool may also wish to take photos & record images of Skillz Skool competitions and programs for promotional purposes. These images may be used in mediums including publications and promotional material, broadcast, print and electronic media. By signing up you also give Skillz Skool, permission to use these images or videos taken of me/my children.
